Generally causes can be due to infection, neoplasia, connective tissue disease or others (like diabetes mellitus). In each the possibility can be high or low (or moderate) depending on its incidence. Therefore, for high probability list for infection can be:  Gall stones (choledocholithiasis), acute cholecystitis, acute appendicitis, chronic pancreatitis. For connective tissue disease Inflammatory bowel disease. Other diseases can be Irritable bowel sydrome, sclerosing cholangiitis. Neoplastic can be lymphoma or colorectal cancer.  Please note that this lists only high incidence diseases. Unless one gives age, gender, history of illness etc., one cannot  be clear.
